Donation form: amount chips, “once / monthly” with ×12
A donation form for a foundation: a photo in a paper frame with a handwritten caption on the left, a form on the right — a segmented “once / monthly” switch, amount chips and a custom amount field. With monthly on, a “×12” panel appears: the yearly total counts up next to a handwritten “that's 12 visits” note. Listens to the vibeui-charity:amount event from the calculator and fills in the chosen amount. After submit a heart and check mark draw themselves. A stub: nothing is sent.
React + Tailwind, shadcn-compatible: press “Copy for AI” and your Cursor or Claude Code agent installs it with the shadcn CLI.

FAQ
- How do I add Donation form: amount chips, “once / monthly” with ×12 with Cursor or Claude Code?
- Press “Copy for AI” on this page and paste it into your agent's chat. The instruction contains the shadcn CLI install command, the dependencies and what to keep, so the agent installs this exact component instead of recreating a lookalike.
- What does Donation form: amount chips, “once / monthly” with ×12 depend on?
- No npm packages beyond React and Tailwind CSS. It also installs catalog components: card-025, button-001.
- What can I change in Donation form: amount chips, “once / monthly” with ×12?
- Adapt: eyebrow, title, lede, caption, image, imageAlt; amounts, defaultAmount, currency, onceLabel, monthlyLabel, monthlyNote ({year}); actionLabel, fine, doneTitle, doneText. Keep as installed: the card-025, button-001 imports from @/components/vibeui/ — do not inline their code into the block or swap in your own; the parts receive props only; the switch moves its ::before with translateX(100%) via data-monthly, buttons carry aria-pressed; the yearly total is a 900 ms requestAnimationFrame from value to value×12 on every change; at once under reduce-motion.
- Is Donation form: amount chips, “once / monthly” with ×12 free?
- The preview is open to everyone; the source is available with a PRO subscription.

Technical notes
A composite block: imports card-025, button-001 from @/components/vibeui/ — the install command brings them along with the block. The block owns the layout and its own text, passes the parts props only and leaves their --vibeui-<name>-* palettes alone.
