Technical notes
The component is self-contained: one file, zero dependencies, its own palette in the local --vibeui-cascader-017-* variables. Client-side: the branch, the speciality and the search string live in useState. The upper level is rendered as chips because there are always three or four branches, while the lower one stayed a list with a filter: specialities run into dozens and a column without search is useless. Filtering is moved into useMemo and works inside the selected branch, so results from different sections do not mix. A speciality with no doctors gets a native disabled and the explanation «not seeing patients» instead of disappearing from the list. The search field is a type="search" with an aria-label of its own that names the current branch.
